Transition Services






Transition Mission: To develop and implement goals, social integration, vocational integration, and inclusion into the mainstream of life, thereby, identifying valued employment and lifestyle outcomes for high school students and completers with disabilities

Role of Parent(s)
  • Knowledge of IEP
  • Knowledge of Transition Placement
  • Knowledge of provider services
  • Communications with Provider(s)
  • Attend transition workshop events
  • Complete student/parent transition surveys
  • Attend and actively participate in all IEP meetings.

Role of the School
  • Help student plan for support and participate in community resources
  • Graduate students with a job that meet community employment guidelines
  • Focus on educating students with peers without disabilities
  • Involve students and families in developing the IEP.
  • Participate in Community Transition Council activities.

Role of Adult Service Provider:
  • Participate in closing review meeting at school.
  • Establishes timeline for transition planning activities.
  • Has regular dialogue with VR counselors
  • Participate in transition IEP meetings
  • Work with school personnel to overlap services during the student’s last year.
  • Participate in Community Transition Council activities
